Position Purpose: Provides evidenced based interventions for patients within the program.  In collaboration with the multidisciplinary healthcare team, the Counselor works collaboratively with the patient’s care team to allow for timely and quality care for mental health and substance use disorders. Position is FLSA exempt. 

Shift/Hours: Days / Monday-Friday

Key Responsibilities: Functions as a consultant, clinician, and mentor in the delivery and evaluation of behavioral health care services to patients within the behavioral integration model of care. May also provide care to other local KRMC locations as needed at the direction of leadership. Performs appropriate assessment and screening for patients referred to the Behavioral Health program. Utilizes the Diagnostic and Statistical Manual of mental health disorders (DSM) current version, in working to identify appropriate diagnosis for patients, and format treatment plans and interventions that align with patient centered goals. Provides motivational interviewing, brief intervention therapy, supportive therapy and counseling, as well as other treatment modalities to patients, families, and groups as deemed by licensure. Collaborates with all members of the patient’s treatment team as appropriate to include providing written reports and or phone consultations and updates. Assists in developing a plan of care, discharge plan, and arrange support and referrals through collaboration with internal staff, providers, and community organizations. Completes timely and appropriate documentation within the EMR system including clinical notes, registry, and collaborative care notes per institution and state requirements. Routinely meets with patient care teams to coordinate patient care, participate in program updates, and provide and acquire staff education for their own professional growth. Qualifications: 

  • Education:  Master’s Degree in the Behavioral Health field in any one of the following: Professional Counselor; Clinical Social Work; Marriage and Family Counselor; Substance Abuse Counselor 
  • Certifications:  BLS certified through American Heart Association or American Red Cross
  • Licensure: 
    • Licensed by the AZ Board of Behavioral Health Examiners, for their licensure
    • DPS Level One Fingerprint Clearance Card
    • Current and valid U.S. driver’s license

Work Requirements: Will require local travel to other KRMC practices and facilities as needed. Potential exposure to bloodborne pathogens and other workplace hazards. Moderate walking, standing, sitting, and computer use throughout the workday. Proficiency with EMR systems, Microsoft applications, and email.
